I got to correct you though, the site never asks you to do a survey or such to be able to use it. It is, like many a website that collects links to other websites hosting the videos, so you may have been asked by one of those sites to do a survey. If that's the case then just close the window and choose another video source. One more thing you need to know when using it is that there is a text link under the first couple of links for a video, click on that one and you will get A LOT more video sources.
